Terms of Participation
Certain programs require a small non-refundable deposit upon submission of this application to hold a reservation. A complete information package for your first session will be forwarded to you after your application and deposit payment have been received and accepted. The materials and concepts of The Growth Coach® program are designed to help you achieve extraordinary results, however the program is not a substitute for the advice of your accountant, lawyer, or any of your advisors, personal or professional. The Growth Coach® reserves the right to decline continued participation for any reason at any time during the course of the program. In the unlikely event of an emergency The Growth Coach® reserves the right to reschedule program sessions and to substitute coaches.
Coaching Program Application Form
I hereby apply for acceptance into The Growth Coach® Program as indicated below. In doing so, I commit myself to full participation in the Program should we agree that the recommended activities meet my objectives as the coachee.
It is important to me that the Coach approach me, the Coachee, with confidence in order for the coaching process to work. If this is evident, everything can happen. My Coach must be emotionally committed to me and possess considerable experience in life situations, business, relationships, ethics, and finances.
The lack of the outlined factors in either party to the process may result in failure in the coaching process. I acknowledge that all of my decisions and actions and the consequence of such decisions and actions, whether made in the context of this program or not, are entirely my own responsibility. I agree to pay for all coaching services in advance or in the month they are rendered. I understand that satisfaction is 100% guaranteed and that all costs less material expenses are fully refundable.
